Chapter 1: Overview

...the purpose of the VIHA Practice Change Implementation Workbook is to provide you with a variety of resources needed for implementing clinical and professional standards as well as practice change.
 
...implementers will employ the suggested resources differently for each implementation effort because each Standard, practice change, organizational context, staff and patient population will vary along with each clinical condition.

...standards may be presented in several different forms such as policies, procedures, protocols, clinical guidelines, care maps, clinical pathways and competencies.

Chapter 2: Get Organized ...to help get you started with your implementation you will want to:
  • Review the Development Work;

  • Identify the Essential Elements - People & Tools;

  • Plan for an Effective Evaluation.

Chapter 3: Create Action Plan ...there are two key documents that you should complete during this phase of the project - the Action Plan and Budget. These documents will assist you in determining the feasibility of the implementation.

During this phase, you and your team will want to complete these tasks:

  • Determine the scope;

  • Identify stakeholders and associated implementation issues;

  • Map out activities and resource requirements;

  • Assemble the action plan;

  • Communicate;

  • Update & report on progress;

  • Proceed with Implementation;

  • Evaluate.

Chapter 4: Implement Action Plan ...when you are ready to implement your action plan, you should be able to clearly answer these questions:
  • What has to be done?

  • Who has to do it?

  • When should it be done?
...you need to continually monitor stakeholder feedback and create contingency plans as required.

Chapter 5: Workbook Wrap-Up ...there are a variety of resources to help you with your implementation. These resources include readings, templates, and checklists. You should select resources that are useful to you.
 
...in the future, you can return to the workbook to refresh your knowledge of implementation and use the many library resources available.
